You drive into a tunnel on a motorcycle. You hit a road block and fly off your bike. As soon as you stand up, the exits of the tunnel have been blocked off, and you see that there is a truck with spikes on the grill, driving toward you. Kill yourself, or die slowly and painfully.

I can't do it again... Yo have to hold left at exactly one certain frame, you should slide through the tiles at the bottom.
